Replacement of Passports Information

IF THE PASSPORT WAS LOST, STOLEN OR DAMAGED THE APPLICANT IS REQUIRED TO SUBMIT THE FOLLOWING:

• Completed Passport Form A for persons sixteen years and older OR
• Completed Passport Form B for children under sixteen years of age
• Two passport size photos
• Barbados birth certificate (original & a copy) for citizens born in Barbados OR Citizenship document if not a citizen by birth
• Barbados ID card (original & a copy). For children under the age of 16 the Barbados ID of the Parent who signs the form is also required.
• If there is a change of name- a change of name document must be presented
• Marriage certificate (original & a copy), females only
• Divorce document - decree absolute (original & a copy), females only
• A report from the appropriate official ( Police Department, Fire Department)
• A letter from applicant explaining how , where and when the passport was lost , stolen or damaged
• The damaged passport if the applicant is in possession of the passport

NB. The applicant will be interviewed in respect of the above-mentioned and other documents may be requested as a result.

The process for collection

• Passports must be collected by the holder of the passport except in special circumstances.
A person can collect on behalf of another person if evidence of the following is produced:

1. The applicant is an elderly person
2. The applicant is incapacitated
3. The applicant suffers from a mental/physical disability or is severly ill
• Persons collecting passports must produce their receipt and ID card or expired passport.

If a passport is being collected by a person other than the Applicant (in special circumstances):

• a letter signed by the applicant giving authorization
• a letter signed and stamped by a Notary Public / Justice of the Peace.

• Id card of person collecting document

The fees for Passports are as follows.  The fees for passports issued by our diplomatic missions overseas vary from mission to mission. You should therefore check with the respective mission regarding the fee schedule.

NB: All Persons over the age of sixteen (16) must submit their application in person when applying for a Barbados passport.

Originals & Photocopies of documents must be submitted

All fees must be paid on submission of applications. Applications will not be accepted without payment of fees.

Minors must accompany parents for passport applications.
